Association of suicide attempts with childhood traumatic experiences in patients with major depression
Öz: Amaç: Çocukluk çağı örselenmelerinin birçok psikiyatrik bozukluk ve özkıyım davranışı gibi çeşitli olumsuz davranış kalıpları için risk oluşturduğu bilinmektedir. Ayrıca, bazı psikiyatrik bozukluklarda da çocukluk örselenmeleri yaşayanlarda özkıyım davranışında artış saptanmaktadır. Bu çalışmada çocukluk örselenmelerinin, majör depresif bozukluktaki özkıyım davranışı üzerine etkisi araştırılmıştır.Yöntem: Çalışmaya majör depresif bozukluk, tam remisyon tanılı, özkıyım girişiminde bulunmuş 30 hasta, majör depresif bozukluk, tam remisyon tanılı, hiç özkıyım girişiminde bulunmamış 30 hasta ve hastalarla yaş, cinsiyet, eğitim açısından eşleştirilmiş 30 sağlıklı kontrol alındı. Tüm olgulara DSM-IV eksen I bozuklukları için yapılandırılmış klinik görüşme (Structural Clinical Interview for DSM-IV/SCID-I) kullanılarak tanı konuldu. Ardından tüm olgulara Çocukluk Örselenme Yaşantıları Ölçeği (ÇÖYÖ) uygulandı.Bulgular: Özkıyım girişiminde bulunmuş depresyon tanılı hastalar; kontrol grubuna ve özkıyım girişiminde bulunmamış hastalara göre ÇÖYÖ’nün duygusal kötüye kullanım ve duygusal ihmal, fiziksel kötüye kullanım, cinsel kötüye kullanım alt ölçeklerinin her üçünden de anlamlı düzeyde daha yüksek puan aldılar. Özkıyım girişiminde bulunmamış depresyon tanılı hastalar ile kontroller arasında ise hiçbir alt ölçek puanında anlamlı fark saptanmadı.Sonuç: Bu bulgularla, çocukluk dönemindeki örselenmelerin depresyonda özkıyım girişimi için risk etmeni olduğu söylenebilir. Depresyon tanılı hastalarda çocukluk örselenmelerinin sorgulanması özkıyım riskinin öngörülmesine önemli katkı sağlayabilir.

Öz: Objective: Childhood traumatic experiences are widely known risk factors in the development of various psychiatric disorders and unfavorable behavior patterns, including suicide attempts. Moreover, in the course of some psychiatric disorders, suicide attempts are more frequent in patients with childhood traumatic experiences. This study explores the influence of childhood traumatic experiences on suicide attempts in patients with major depression. Method: Our study involved 30 patients with major depressive disorder in full remission who had attempted suicide, and 30 patients with major depressive disorder in full remission who had never attempted suicide, as well as 30 healthy controls matched with patients for age, gender, and education. All the subjects were interviewed with Structural Clinical Interview for DSM-IV (SCID-I). Subsequently, all subjects were administered Childhood Trauma Questionnaire (CTQ). Results: Patients who had attempted suicide reported significantly higher CTQ scores for emotional abuse and emotional neglect, physical abuse, and sexual abuse than healthy controls. Moreover, patients who had attempted suicide reported significantly higher CTQ scores for each three sub- domains than patients who had never attempted suicide. There were no significant difference between patients who had never attempted suicide and healthy controls in any of the CTQ subdomains. Conclusion: Childhood trauma may be a risk factor for suicide attempts in patients with depression. Inquiring childhood traumatic experiences could make an important contribution to predict suicide risk in patients with depression.
