Diş Hekimliğinde Akılcı Antibiyotik Kullanımı

Öz:
Başka pek çok ülkede olduu gibi, Türkiye'de de antibiyotiklerin gereksiz ya da yanlış kulla-- nımları ve buna bağlı akılcı olmayan antibiyotik kullanımı sorunu yaygındır. Antibiyotikler diş he-- kimlerinin reçete ettiği ilaçlar arasında da önemli yer tutmaktadır. Antibiyotiklerin akılcı olmayan kullanımıyla ortaya çıkan sorunlar güncel tartışma konuları arasında yer almakta ve ilgili çevre-- lerce çözüm önerileri üzerinde durulmaktadır. Dental enfeksiyonların tedavi ve profilaksisinin akılcı ilaç kullanımı (AİK) genel ilkelerine uyularak düzenlenmesi yol göstericidir. AİK, "ilaçların hastaların klinik ihtiyaçlarına uygun şekilde, uygun dozlarda, yeterli sürede, kendilerine ve top-- luma en düşük maliyette almaları" olarak tanımlanmaktadır. Bu çerçeve içerisinde antibiyotikle-- rin akılcı kullanımında diş hekimlerine önemli görevler düşmektedir. Diş hekimi, öncelikle bu ilaçlar konusunda yeterli farmakoloji bilgisine sahip olmalı, antibiyotikleri gelişigüzel biçimde uy-- gunsuz kullanmamaya özen göstermeli, antibiyotik seçimini "kişisel tedavi" ve "kişisel ilaç" yak-- laşımıyla her bir endikasyonda belirlemeli, reçetelemeyi planladığı antibiyotiğin hastaya uygunluğunu değerlendirmeli, ilaç dışındaki diğer tedavi işlemlerini ve tavsiyelerini eksiksiz yap-- malı, doğru format ve içerikte reçete yazmalı, hastayı yeterince bilgilendirmeli, süreci takip ede-- rek başarıyla yönetmeli ve hastayı tedavinin ortak katılımcısı yapmalıdır. Hastaya veya dental işlemlere bağlı özel durumlar söz konusu olduğunda, diş hekimlerinin AİK penceresinden bu ko-- nuların her birine özgü yeterli bilgi ve beceriye önceden sahip olması ve bunun için hazırlıklı ol-- ması gerekmektedir.

The Rational Use of Antibiotics in Dentistry: Review

Öz:
Irrational use of antibiotics associated with unnecessary or improper usage, is also a common problem in Turkey as reported in many countries. Antibiotics, take an important place among dentist prescriptions. The problem of irrational use of antibiotics is situated between the cur-rent debate and related authorities focused on solutions. Following the general principles of rational use of medicine (RUM) is guide to treatment and prophylaxis of dental infections. RUM is defined as "patients receive medications appropriate to their clinical needs, in doses that meet their own individual requirements, for an adequate period of time, and at the lowest cost to them and their community". In this viewpoint’ the dentist has an important role for the rational use of an- tibiotics. Primarily the dentist should have sufficient knowledge of pharmacology about these med- ications and take care not to use antibiotics inattentively and inappropriately. The choice of antibiotic for every indication should be based on “personal treatment’’, “personal medication’’ ap- proaches. The dentist intends to prescribe antibiotics should assess the suitability of patients. All other treatment procedures and recommendations out of the drug should be made completely. The prescription should be written at correct format and content. The patient should be informed about the antibiotic treatment sufficiently. The dentist must successfully managed the process and make the patient common participants of the treatment. Due to special conditions in the case of the pa- tient or dental procedures, the dentist must have sufficient knowledge and skills about each issue of RUM previously
