Çölyak Hastalığı ve Glutensiz Diyet Tedavisine Güncel Yaklaşım

Öz:
Çölyak hastalığı; genetik olarak yatkınlığı olan bireylerde, otoimmün mekanizmalarla, başlıca buğdaydaki gluten ve arpa, çavdar, yulaf gibi tahıllardaki gluten benzeri diğer tahıl proteinlerine karşı kalıcı intolerans olarak gelişen proksimal ince barsak hastalığıdır. Gluten, gliadin veglutenin olmak üzere iki temel fraksiyondan oluşmaktadır. Çölyak hastalığı ile ilişkili olan proteinfraksiyonu gliadindir. Serolojik testler ve barsak biyopsisi gibi modern tanı yöntemlerinin artmasıyla toplumlarda çölyak prevalansını saptamak kolaylaşmıştır. Çölyak hastalığında mukozal hasarın derecesini ve ince barsak patolojisini değerlendirmede ″Modifiye Marsh Kriterleri″ referansalınmaktadır. Çölyak patogenezinde genetik, çevresel ve immünolojik faktörler rol oynamaktadır.Çölyak hastalığının gelişiminden sorumlu genlerin insan lökosit antijenleri ile güçlü bir bağlantısıbulunmaktadır. Günümüzde çölyak hastalığının tek tedavisi glutensiz diyet uygulamasıdır ve hastalar ömür boyu bu diyeti uygulamak zorundadır. Yeni tanı almış çölyak hastalarının nütrisyoneldurumları, gastrointestinal sistem hasarının boyutu, malabsorpsiyon derecesi ve bireyin tanı konmadan önceki aktif hastalık dönemine bağlı olarak değişkenlik göstermektedir. Bu nedenle glutensiz diyet uygulayacak hastanın beslenme programı bir diyetisyen tarafından ayrıntılı şekildeyazılmalı, hastalar belirli aralıklarla değerlendirilmeli ve yaşam boyu izlem altında olmalıdır.

Celiac Disease and the Contemporary Approach to Gluten-Free Diet Treatment

Celiac disease is a proximal small intestine disorder which occurs in autoimmune mechanisms of genetically tendent individuals, causing a permanent intolerance against gluten in wheat and gluten-like proteins in rye, barley, oat etc. Gluten consists of two basic fractions, namely gliadin and glutenin. The protein fraction related to the celiac disease is gliadin. Due to the increase of the modern diagnostic methods such as serological tests and colonel biopsy, detecting the celiac prevalence in society got easier. For evaluation of mucosal damage amount and small intestine pathology about celiac disease, “Modified Marsh Criteria” are taken reference. Genetic, environmental and immunological factors take part in celiac pathogenesis. The genes which are responsible for celiac disease have a strong relation with human leukocyte antigens (HLA). At the present time, the only treatment to celiac disease is gluten-free diet and patients are obliged to have this diet for their lifetime. The nutritional status of newly diagnosed celiac patients varies in accordance with the gastrointestinal system damage amount, malabsorbtion rate and individual’s time spent in active disease period before the diagnosis. Therefore gluten-free diet ought to be prepared in detail by a dietitian, patients ought to be evaluated within specific periods and be watched over for their lifetime.
