Prematür Yenidoğan Kanaması

Öz:
Periventriküler-intraventriküler hemoraji özellikle 32. gestasyonel haftadan daha önce doğan prematürleri etkileyen kompleks birdurumdur. Bu hastalarda artan mortalite oranları ile birlikte prematür doğumun diğer morbiditeleride daha sık görülür. Prematür yenidoğan kanaması düşük doğum ağırlıklı pretermlerde serebral hasarın en önemli sebebidir. Kanama periventriküler alanda yerleşengerminal matriks kapiller ağından olur. Kanama takiben ventriküle açılarak intraventriküler hemorajiye döner. Önemli nöro-gelişimselproblemler ve mortalite riski mevcuttur. Ciddi kanamalarda yüksek oranda post-hemorajik hidrosefali ve periventriküler lökomalazigelişir. Çalışmada prematür infantlarda ortaya çıkan intraventriküler hemorajinin altında yatan mekanizmalar tanımlanmış, hastalıktankorunma yöntemleri irdelenmiştir. Kanama ortaya çıktığında sınırlayıcı bir tedavi olmamakla birlikte eskiden beri uygulanan klasikfarmakolojik ve cerrahi tedavi seçenekleri, komplikasyonlar ile birlikte gözden geçirilmiştir.

Periventricular-intraventricular hemorrhage is a disease process that affects premature newborn infants, especially those born with a gestational age of less than 32 weeks. Hemorrhage occurs from the germinal matrix in the periventricular area that can then extend into the ventricles as intraventricular hemorrhage. Severe hemorrhage usually creates a risk for both post-hemorrhagic hydrocephalus and periventricular leukomalacia associated with high degrees of significant neurodevelopmental disability and mortality. This review broadly describes the mechanism underlying intraventricular hemorrhage and assesses the whole treatment and management strategies, pharmacologic and surgical modalities, and the techniques and their complications in the management of intraventricular hemorrhage and post-hemorrhagic hydrocephalus in premature infants
