Ankilozan spondilit ve psöriyatik artritte sekukinumab deneyimi: Tek merkezden 44 hastanın analizi

Ankilozan spondilit ve psöriyatik artritte sekukinumab deneyimi: Tek merkezden 44 hastanın analizi
Öz: Amaç: Sekukunimab IL-17A’ya bağlanan IgG1-kappa tam insan
monoklonal antikorudur ve uzun dönem verileri bilinmektedir.
Ülkemizde ise tedavi sonuçlarıyla ilgili gerçek yaşam verisi kısıtlıdır.
Çalışmamızda ülkemizde görece yeni kullanıma giren sekukinumabın
kliniğimizden takipli ankilozan spondilit (AS) ve psöriyatik artrit (PsA)
hastalarındaki erken dönem sonuçlarını değerlendirmeyi amaçladık.
Yöntem: Çalışmamıza Mayıs 2018-Ekim 2019 tarihleri arasında
sekukinumab tedavisi başlanan AS ve PsA tanılarıyla polikliniğimizden
izlenen 44 hasta dahil edilmiş olup verileri retrospektif olarak
incelenmiştir.
Bulgular: Çalışmaya dahil edilen 44 hastanın ortalama yaşı 44,9
olup çoğunluğu erkekti (%61,4). Otuz üç AS (%75) ve 11 (%25) PsA
hastasında sekukinumab tedavisine başlandı. Sekukinumab tedavi
süresi AS’de ortalama 9,1±5,3 ay, PsA’da ise ortalama 7,0±4,0 aydır.
Sekukinumab devam oranı AS’de %75,7 (n=25) olurken PsA’da
%63,6 (n=7) olarak gerçekleşmiştir. Sekukinumab tedavisi altında ciddi
enfeksiyon veya malignite gelişimine takip süresince rastlanmamıştır.
Sonuç: Sekukinumab AS ve PsA’da anti-TNF kullanmamış ya da
yanıtsızlık ve/veya intolerans nedeniyle kullanamayan seçilmiş
hastalarda etkin ve güvenilir bir tedavi seçeneği olarak görünmektedir.

Öz: Objective: Secukunimab is a fully human monoclonal antibody
to IgG1-kappa that binds to IL-17A and its long-term data are well
known. However, real-life data on treatment outcomes are limited in
our country. In our study, we aimed to evaluate the early results of
secukinumab, which is relatively newly used in our country, in patients
with ankylosing spondylitis (AS) and psoriatic arthritis (PsA), who were
followed up in our clinic.
Methods: Forty-four patients were included in the study with
the diagnosis of AS and PsA. Secukinumab treatment was started
between May 2018 and October 2019, and their data were analyzed
retrospectively.
Results: The mean age of patients was 44.9 years and most of them
were male (61.4%). Secukinumab treatment was initiated in 33 AS
(75%) and 11 (25%) PsA patients. The mean duration of secukinumab
treatment was 9.1±5.3 months in AS and 7.0±4.0 months in PsA.
The continuation rate of secukinumab was 75.7% (n=25) in AS and
63.6% (n=7) in PsA. No severe infection or malignancy development
was observed during secukinumab treatment in the follow-up period.
Conclusion: Secukinumab appears to be a safe and effective
treatment option in selected patients with treatment naive to anti-
TNFs or in patients with AS and PsA who could not use it due to
unresponsiveness and/or intolerance.
