Sepsis, Lökositler, Sitokinler ve Dissemine İntravasküler koagukasyon

Öz:
Lökositler, sepsiste gelişen Dissemine İntravasküler Koagulasyon (DİK)'un patogenezinde önemli rol oynamaktadırlar. Lökositler konakçı savunmasındaki faydalı rollerine rağmen ayrıca sepsiste doku hasarının başlaması ve ilerlemesine de sebep olmaktadırlar. Tümör Nekrozis Faktör- a (TNF-α) ve İnterlöykin- 1β (IL-1 β) gibi sitokinler monosit, nötrofil ve endotel hücreleri aktifleştirmektedirler. Yangısel sitokinler endotelyumu antitrombotik yüzeyden protrombotik yüzeye dönüştürmeyle koagulasyon mekanizmasını başlatmaktadırlar. Sözkonusu sitokinler doku faktörü oluşumunu stimüle ederek ekstrinsik ve intrinsik koagulasyon sistemini aktive etmektedirler. Bu sitokinler ayrıca nötrofilleri aktifleştirmekte ve nötrofil elestaz ve oksijen serbest radikalleri gibi yangısel mediyatörleri salmaktadırlar. Sepsiste gelişen çeşitli patojenik etkiler nötrofillerden salınan oksijen radikalleri ve enzimlere bağlanmaktadır. Mevcut derleme sepsiste lökositler ve önemli rolleri hakkında bilgi vermektedir.

Sepsis, Leukocytes, Cytokines and Disseminated Intravascular coagulation

Öz:
Leukocytes play an important role on the pathogenesis of sepsis-induced disseminated intravascular coagulation. Despite the beneficial role of leukocytes in host defence, these cells may also cause the initation and progression of tissue injury in sepsis. Cytokines such as tumor necrosis factor- oc (TNF-α) and interleukin-1 β (IL-1 β) activate monocytes, neutrophils and endothelial cells. Inflammatory cytokines initate coagulation mechanism by converting endothelium from an antithrombotic surface to a prothrombotic surface. These cytokines activate both the extrinsic and intrinsic coagulation system by stimulating tissue factor production. Moreover, they activate neutrophils to release inflammatory mediators such as neutrophil elestase and oxygen free radicals. Various pathogenic effects that occur in sepsis are attributed to the release of oxygen radicals and enzymes from neutrophils. This review present information about leukocytes and their important roles in sepsis.
