Epilepsinin Gelişimsel Süreçte Travma ve PsikojenikNöbet ile İlişkisi

Öz:
Epilepsi nöbeti, ani ortaya çıkması, işlevselliği bozması, kazalara ve yaralanmalara neden olabilmesi ve ölüm korkusu yaşanması nedeniyle travmatize edici olabilmektedir. Epileptik nöbetlerin kendisi travmatize edici olabildiği gibi, özellikle geçirilen kafa travması sonrasında da epilepsi ortaya çıkabilmektedir. Çocuklukta başlayan epilepsinin şiddeti ve süresi arttıkça gelişimi bozucu etkisi ortaya çıkmaktadır. Epileptik olgularda yıllar içinde konversif (psikojenik) nöbetler de tabloya eklenebilmektedir. Psikojenik nöbeti olan olguların yaklaşık yarısında psikolojik travma ile ilişkili olarak ortaya çıkan dissosiyatif bozukluk eştanısı görülmektedir. Genel olarak kronik hastalığı olan çocuklarda hastalıkla lişkilitravmatik etki ile dissosiyatif bozukluk oluştuğu bilinmektedir. Dissosiyatif bozuklukta konversiyon bozukluğu ve konversif bayılmalara da sık rastlanmaktadır. Bir kronik hastalık olan epilepside travma sonrası stres bozukluğu (TSSB), dissosiyatif bozukluk ve psikojenik nöbet eştanısı sık olarak görülmektedir. Bu nedenle epilepsi olgularını ele alırken travmatik etki ve ilişkili dissosiyatif bozukluk boyutu psikiyatrik yaklaşımda yer almalıdır. (Nö­rop­si­ki­ yat­ri Ar­fli­vi 2013; 50: 291-294)

Öz:
An epileptic seizure, can cause trauma for its sudden emergence, leading to functionampairment, accidents and injuries, and fear of death. The seizure can be traumatizintself, besides, an head trauma that may occur during the seizure can also cause epilepsyAs the severity and duration of epilepsy increases, disturbances in development anraumatic effects occur. Conversion (psychogenic) seizures may be added over thyears in epileptic patients. The comorbidity of trauma-related dissociative disordeand psychogenic seizures is observed in approximately half of the cases. Dissociativdisorders are known to occur in children with chronic diseases due to the traumatieffect of the disease. Conversion disorder and psychogenic seizures are frequentlseen in dissociative disorders. Posttraumatic stress disorder, dissociative disorders, anpsychogenic seizures are often comorbid diagnoses in epilepsy. For this reason, whestudying epilepsy, traumatic effect and associated dissociative disorder dimension oepilepsy should be kept in mind in the psychiatric approach.
