DİSTRİBUTİON OF HEPATİTİS C VİRUS GENOTYPES İN İNJECTİON DRUG USERS WİTH CHRONİC HEPATİTİS C

Öz:
Amaç: Hepatit C virüsü çeşitli yollarla bulaşabilmesine rağmen, gerek tüm dünyada gerekse belli bölgelerde HCV enfeksiyonunun yayılmasında rol oynayan en önemli risk faktörü damar içi uyuşturucu madde kullanıcılarıdır (DİUMK).Ülkemizde HCV ile kronik enfekte DİUMK'larda yapılmış çalışma sayısı oldukça kısıtlıdır. Bu çalışmada kronik HCV enfeksiyonlu damar içi uyu- şturucu madde kullanıcılarında HCV genotiplerinin dağılımını saptamak amaçlanmıştır. Gereç ve Yöntemler: Çalışma grubuna Mersin Ünivesitesi Tıp Fakültesi, Klinik Mikrobiyoloji ve Enfeksiyon Hastalıkları bölümüne başvuran HCV ile enfekte DİUMK'lar dahil edildi. Serolojik testler ELİSA yöntemi ile çalışıldı. HCV enfeksiyonu HCV RNA saptanması ile doğrulandı. HCV RNA izolasyonundan (MagNA Pure LC Total Nucleic Acid Isolation Kit, Roche) sonra, viral yük tayini COBAS TaqMan 48 (Roche Diagnostic, ABD) prosedürüne göre "real time" polimeraz zincir reaksiyonu (RT-PCR) tekniği ile gerçekleştirildi. HCV genotiplerinin belirlenmesi, AMPLIQUALITY HCV-TS (AB Analitica) test kiti yöntemi kullanılarak yapıldı. Bulgular: Kronik HCV enfeksiyonlu 238 DİUMK'nın 120'sinin (%50,4) genotip 1 ile enfekte olduğu gözlendi. Bunların112'si (%93,3) subtip1a ve 8'i (%6,7) subtip 1b ile enfekteydi. Genotip 3, olguların 81'inde (bütün olguların %34'ü), genotip 2, 22'sinde (bütün olguların %9,3'ü) ve genotip 4, olguların 15'inde (bütün olguların %6,3'ü) saptandı. Genotip 5 ve 6'ya çalışma grubunda rastlanmadı. Çalışmada HCV genotipi ve hastaların yaşı ve serum HCV RNA düzeyleri arasında bir ilişki saptanmadı. Sonuç: Çalışmamızda genotip subtip 1a ve genotip 3'ün bu zor hasta grubunda egemen genotip olduğu gösterilmiştir.

Kronik Hepatit C Enfeksiyonlu Damar İçi Uyuşturucu Madde Kullanıcılarında Hepatit C Virüs Genotiplerinin Dağılımı

Öz:
Objective: Although there are many route for HCV transmission, currently the major contributor and driving risk factor for localized and universal spreading of HCV infection are injection drug users (IDUs). In our country the number of studies concerning IDUs with chronic hepatitis C is very scantly. In this study we aimed to determine the distribution of various genotypes of HCV in IDUs with chronic HCV infection. Material and Methods: The study group included IDUs recruited at Mersin University, department of Clinical Microbiology and Infectious Diseases. Serological tests were done ELISA method. HCV infection was confirmed by presence of HCV RNA. After the HCV-RNA isolation (MagNA Pure LC Total Nucleic Acid Isolation Kit, Roche), viral load was determined according to COBAS TaqMan 48 (Roche Diagnostic, ABD) procedure. This was performed with “real time” polymerase chain reaction (RT-PCR) technique. HCV genotypes were studied by AMPLIQUALITY HCV-TS (AB Analitica) kits Results: Genotype 1 was observed in 120 of the 238 IDUs (50.4%) with chronic HCV infection. Of these, 112 IDUs showed infection with subtype 1a (93.3%) and 8 with subtype 1b (6.7%). Genotype 3 was determined in 81 IDUs (34% of all cases), genotype 2 in 22 (9.3% of all cases), and genotype 4 in 15 IDUs (6.3% of all cases). Genotypes 5 and 6 were not found in the study population. There was no association of HCV genotype with patient age and serum HCV RNA levels. Conclusion: This study demonstrated that genotype subtype 1a and genotype 3 are predominant genotypes in our troublesome population
